Ron,
As we discussed by phone earlier this week, here is a summary of the status of the Kern
demolition project, evidence gathering and the RCA related to the 2013 incident:
? The Kern Power Plant demolition is approximately 45% complete;
? PG&E's current target date for completing demolition and retrieving all evidence is
December 4th;
? Evidence is being photographed, tagged and stored pursuant to the protocol for retrieval
and storing of evidence; approximately 70% of the evidence has been collected to date;
? PG&E is currently developing a joint inspection protocol and agreement with parties to
govern the process for examining and conducting destructive lab testing of the physical
evidence;
? Once this agreement is finalized and all evidence is retrieved and secured, parties and
Cal/OSHA will begin their respective technical evaluations of the evidence;
? Exponent has been retained to determine the direct cause (i.e., technical cause) of the
incident. PG&E will work with Exponent on a schedule for completing the direct cause
analysis once all the evidence has been collected and a schedule for access to evidence
has been developed with OSHA;
? PG&E also plans to retain Exponent to prepare a Root Cause Analysis (RCA) and use the
direct cause analysis as an input into this RCA. PG&E anticipates executing a new work
order with Exponent soon regarding preparation of the RCA. Exponent anticipates it will
take approximately 8 weeks to prepare the RCA after it completes the direct cause
analysis.
I will send you the RCA work order once it is executed with Exponent.
